* Are you taking multiple medications? Our pharmacist will review your

medication to ensure that you are safe and getting the most out of your

medication.


PRESCRIPTION TRANSFERS 


* Transferring your prescriptions to us is quick and easy. 

* Fill out a transfer request online to have your prescriptions transferred

from your last pharmacy.

PRESCRIPTION REFILLS


* Submit your refill request online to easily order your prescription refills.

* Use the "Select-a-Service" drop-down.


PRESCRIPTION RENEWALS


* Have a pharmacist renew a prescription or provide you with a refill extension

if deemed appropriate, to help you avoid any interruptions

COMPOUNDING PRESCRIPTIONS


* If your medications are not commercially available in the dose or form they

were prescribed, we can create the medication in a form or dose for you.


MINOR AILMENTS


As of January 1, 2023, as per regulations your City Life pharmacist will be

authorized and ready to prescribe medications for 13 minor ailments that

include:


* Allergic rhinitis

* Candidal stomatitis (oral thrush)

* Conjunctivitis (bacterial, allergic and viral)

* Dermatitis (atopic, eczema, allergic and contact)

* Dysmenorrhea

* Gastroesophageal reflux disease (GERD)

* Hemorrhoids

* Herpes labialis (cold sores)

* Impetigo

* Insect bites and urticaria (hives)

* Tick bites, post-exposure prophylaxis to prevent Lyme disease

* Musculoskeletal sprains and strains

* Urinary tract infections (uncomplicated)


SHARPS DISPOSAL PROGRAM 


* Safe disposal of syringes, pen needles, lancets, insulin, pump infusion

supplies, and test strips


MEDICATIONS RETURN PROGRAM


* Safe disposal of expired or unused medications, creams, and inhalers
