Twin-To-Twin Transfusion Syndrome (TTTS)

Twin-to-twin transfusion syndrome, also known as TTTS, is a rare condition that can occur in identical twins who share a placenta. This occurs when blood vessels in the placenta connect the twins' circulatory systems in an unequal manner. This can lead to serious complications for both babies if not treated promptly.

Symptoms:

  • Polyhydramnios in one twin and oligohydramnios in the other
  • Rapid weight gain in one twin and slow growth in the other
  • Heart defect in one twin
  • Premature birth

Diagnosis:

TTTS is usually diagnosed during a routine ultrasound exam. The doctor may observe differences in the amniotic fluid levels around each twin and differences in the size of each twin. Additional tests, such as Doppler ultrasound and fetal echocardiogram, may be performed to confirm the diagnosis.

Causes:

The exact cause of TTTS is not fully understood. It is believed to be due to the connections between the blood vessels in the placenta being unbalanced, leading to one twin receiving too much blood flow and the other receiving too little.

Risk Factors:

  • Having identical twins
  • Sharing a placenta
  • Previous history of TTTS
  • Maternal age over 35

Treatment:

Treatment for TTTS depends on the severity of the condition and the gestational age of the twins. In some cases, if TTTS is discovered early, a procedure called laser ablation may be performed to seal off the blood vessels that are causing the imbalance. In more severe cases, fetal intervention or early delivery may be necessary.

Prevention:

Although there is no guaranteed way to prevent TTTS, there are steps that can be taken to reduce the risk:

  • Regular prenatal care
  • Avoiding smoking and alcohol consumption during pregnancy
  • Monitoring amniotic fluid levels during routine ultrasounds

In conclusion, TTTS is a serious condition that requires prompt diagnosis and treatment to ensure the best possible outcome for both babies. If you suspect that you may be at risk for TTTS, it is important to discuss your concerns with your healthcare provider and follow their recommendations for monitoring and treatment.
