New research suggests your heart registers all of it, and that light in your bedroom at night may be associated with measurable changes to the organ's structure.

A study published this week in the European Heart Journal found that light exposure during nighttime sleep is linked to potentially harmful changes in both the structure and function of the heart, in an analysis of 11,071 UK Biobank participants who had no cardiovascular disease at the outset. The work was led by Professor Lu Qi of Tulane University in New Orleans. Whether light in your bedroom at night is doing this to you is not something a single study settles, and the fixes are cheap enough that the question is worth ten minutes.

What does nighttime light exposure do to the heart?

Nighttime light exposure was measured individually rather than by questionnaire, which is what makes this unusual. Participants wore a wrist device with an integrated light sensor for seven days between 2013 and 2015. Roughly three years later, they underwent cardiovascular magnetic resonance imaging so researchers could examine the structure and performance of the heart directly.

Researchers then compared people exposed to more than three lux at night against people exposed to almost none. The higher-exposure group showed thickening of the wall of the left ventricle, the heart's main pumping chamber, which reduced the space inside it, along with signs that the muscle's ability to flex during a heartbeat was reduced. That is an early indicator of dysfunction. Changes also showed up in the right ventricle and left atrium.

How much light is too much at night?

How much light is too much has a surprisingly low answer. Three lux is not much light at all. Qi has named streetlight leaking through window shades, clock radio illumination and the LEDs on equipment like air purifiers or humidifiers as realistic sources in the study population. The comparison group that fared better was the one exposed to almost no light at all, so the practical target is as close to dark as your room will get.

Why is cardiac remodelling the finding that matters?

The mechanism appears to run partly through sleep. Qi has said the effect may work in part by shortening sleep duration and quality. Nighttime light also disrupts the circadian rhythm, the internal clock that helps regulate blood pressure, heart rate and stress hormones. Earlier work had already linked nighttime light to higher risks of coronary artery disease, heart attack, heart failure and stroke. This is the first study to look at what it does to the heart's structure itself, a process clinicians call cardiac remodelling, typically seen in response to chronic stress or injury to the heart.

Some caution is warranted before you panic about your alarm clock. This is an observational study, so it shows an association rather than proof that the light caused the changes, and light was measured at the wrist rather than at the eyes. The participants were UK Biobank volunteers, who are not a perfect stand-in for the Canadian population. If light is not the issue and you are still waking unrefreshed, the pattern behind waking up gasping for air and still being exhausted after eight hours is a more likely explanation and needs its own assessment.
